I, the undersigned, understand that this is a "No Frills" grooming facility. Therefore, I realize that there is not a professional groomer at this facility. I also understand that the facility cannot guarantee specific show cuts or lengths.

I, the undersigned, do hereby certify that I am the owner (or duly authorized agent for the owner) of the animal described above, that I do hereby give Cynthia Davis, DVM, her staff, agents, and/or representatives full and complete authority to perform the grooming/treatment procedure described. I do hereby forever release the said Doctor, staff, agents, servants and/or representatives from any and all liability arising from said treatment on said animal.

I authorize Tram Road Animal Hospital to take before and after pictures of my pet to display on their Facebook page as well as in their Before and After grooming photo album.
